E-Verify submission fields
Here are all possible fields that may be included in the E-Verify submission.

| Duplicate Reason | The reason that a duplicate case is being submitted for this person. 255 Character limit. |
| First Name | Employee first name |
| Middle Initial | Employee middle initial. Leave blank if none. |
| Last Name | Employee last name |
| Other Last Names Used | Any last names that the employee has used in the past. For example, a maiden name. Leave blank if none. |
| Date of Birth | Employee date of birth. |
| Email Address | Employee email address. Leave blank if none. |
| Phone Number | Employee phone number. Leave blank if none. |
| SSN | Employee Social Security Number |
| Alien Number | Employee Alien Number. |
| Citizenship Status | Employee citizenship status. |
| List A Document | List A Document used on the Form I-9. |
| List B Document | List B Document used on the Form I-9. |
| List C Document | List B Document used on the Form I-9. |
| Document Subtype | Provide more details on the type of document that has been supplied, leave blank is there are no options. |
| I-94 Number | I-94 Number used on the Form I-9. |
| I-551 Number | I-551 Number used on the Form I-9. |
| I-766 Number | I-766 Number used on the Form I-9. |
| US Passport Number | US Passport Number used on the Form I-9. |
| Foreign Passport Number | Foreign Passport Number used on the Form I-9. |
| Document Number | Document Number used in list B or C the Form I-9. Tip: If you entered a Resident Alien Card number that is Pre-1989, the system might loop you back to the Initiate Verification page. If this happens, remove the number in the Document Number field on the Initiate Verification page and retry. |
| Document Expiration Date | Expiration date of the document used on the Form I-9. |

| Visa Number | Visa Number used on the Form I-9. |
| Date of Hire | First day of employment for pay from the Form I-9. |
| Reason for Delay | Selection of reasons that submission of the E-Verify case is more than 3 days past the hire date. |
| Reason for Delay Description | When "Other" is selected as the reason for delay a description must be supplied. In the case of a government shutdown, USCIS has previously stated "E-Verify Not Available" should be used. |
| SEVIS Number | Employees Student and Exchange Visitor Information System number. |
