You can find all available self-service Forms in the top section of the Forms tab.

		Description
1	Form Name	The name of the Form.
2	Form Description	A description of the Form (what the Form does/collects).
3	Action icons	Preview a self-service Form. Edit a self-service Form. Rollback a Form. Rollback means going back one version. Delete a self-service Form. Publish a self-service Form. (A grayed out check mark means the Form is not published yet.) View self-service Form version history
