Add a checkbox question to a survey

  1. On the Survey Question Properties page, in the Question box, type the question.Select the Required check box if the question is required.
  2. In the Type drop-down list, select Checkbox (Yes/No). The checkbox (yes/no) fields appear.
  3. Complete the fields.
  4. Click Add.
