Create a manager requested feedback form template

• Rival Perform comes with a default manager requested feedback form (without ratings). You can create a new version to meet your organization's needs. The new version is not available for use until it is published.
• The default feedback form is available in all of the supported languages. If you choose to create a new version, you must make any textual changes in that language. There is no automatic translation. It's all on you.

1. From the navigation bar, click Administration.From the Settings page under Appearance and Customization, click Form Templates. The Form Templates page opens.
2. Click the Feedback tab.
3. Select the language for this template.
4. Beside the manager requested feedback template, click and then click Create New Version.
5. Make changes using the eForm Builder.
6. Click Save. The new version is saved as a draft. When you are ready for everyone to use it, click >Publish.
